KEIC loss runs are provided upon request
of the producing agent or the named insureds,
via fax. Please submit your fax request on company letterhead
to KEIC Loss Report Request at 206/269-8905.
Please include the following information:
- Insured name(s)
- Policy number(s)
- Policy dates or years
- Valuation date
- Broker of record letter (if requestor is a broker
but not the broker at time of coverage with KEIC)
- Signature of company officer (if requestor is the
insured)
